How to Request a Change of Name
To update your name, please complete our Update Contact Details form.
You will be asked to provide:
Your Test ID and one other identifier that is not your Name
OR two identifiers that are not your Name
This allows us to verify your identity before updating your details.
Under the dropdown "Select the detail you would like to change", choose "Name" to update your name.
