If your query requires input from your healthcare professional (such as your midwife or GP).

How to Request This

To request that we contact your healthcare professional on your behalf, please complete our Referral to Healthcare Professional form.

You will be asked to provide:

• Your Test ID

• OR your postcode and phone number (if you do not have your Test ID)

This allows us to locate your record and process your request.

Information We May Need

Please provide as much detail as possible, including:

• The reason for contacting your healthcare professional

• Any relevant details that will help them support you

What Happens Next

A member of the Customer Care team will review your request and contact your healthcare professional where appropriate.

You may be asked to complete a short security check before any information is shared.
